Currently this was the first I could find http://www.pedalhaus.com/e31.html I have never dealt with them but they have appear to have been around for a while.
Ebay used to have a fair number of AC Schnitzer replica dead pedals.
The problem is that most are in pedal sets but appearing to be flat it shouldn't be hard to make your own

I find this to be either a joke post, or the LHD cars are different from the RHD cars.
LHD cars have a footrest from the factory. -
... but LHD cars have it on the left, outer side of the car where there is nothing in the way.
In RHD cars you have next to your left foot the gear box tunnel widening up towards the fire wall AND additionally the brake linkage eating up plenty of the remaining space towards the front.The difference between a RHD and a LHD car is almost like breaking your legs in a Lambo
Not funny but a classical engineering/ergonomic oversight and really not even enough space for a proper footrest.Reinhard
